Ingineria celulelor T, reprogamată pentru a exprima receptori de antigen chimerici (CAR) sau receptori de celule T (TCR), a revoluționat tratamentul cancerului și este explorată ca terapie pentru boli autoimune și infecțioase, potrivit nature.com. Îmbunătățirea funcției celulelor T prin editarea genelor a arătat promisiuni considerabile.

Cu toate acestea, procesul de fabricație ex vivo este lung și costisitor, limitând accesibilitatea acestor terapii. Generarea in vivo a celulelor T CAR ar putea depăși aceste obstacole, dar metodele actuale se bazează fie pe expresia tranzitorie, cu durabilitate limitată, fie pe integrarea aleatorie a pachetelor de ADN, care nu au specificitate. Aici demonstrăm că expresia stabilă și specifică pentru celule a transgenelor poate fi obținută prin integrarea specifică in vivo a pachetelor mari de ADN.

Am dezvoltat un sistem cu doi vectori pentru a livra ribonucleoproteine CRISPR–Cas9 și un șablon de ADN donator, folosind vehicule de livrare învelite și viruși adeno-asociați, respectiv. Am optimizat ambii vectori pentru livrarea specifică celulelor T și eficiența țintirii genice. Prin integrarea unui transgen CAR într-un loc specific pentru celule T, generăm niveluri terapeutice de celule T CAR in vivo în modele de șoareci umanizați cu aplazie de celule B și malignități hematologice și solide. Aceste descoperiri oferă o cale către terapii cu celule T mai eficiente, precise și accesibile pe scară largă.
